Major Responsibilities:
In collaboration with AMG administrative leadership, is responsible for the design, performance, and workforce planning for Senior Services. Develops strategies to optimize performance and align with AH’s strategic plan and contributes to its success measures in patient experience, clinical quality, and financial performance. Partners with Regional and Area leadership teams to integrate Senior Services strategies and growth initiatives into local plans.
Responsible for deployment of evidence-based best practices to clinicians in Senior Services and supports clinical decision-making through development or sourcing of solutions to promote clinician adoption and adherence.
Leads education and training of physicians, advanced practice clinicians and nurses on new initiatives and monitors delivery and quality of care. Develops and implements peer review and quality improvement audits for initiatives within the scope of this role.
Collaborates with clinical and business leaders to advance their strategies or initiatives when Senior Services is a component.
Provide leadership, mentoring, teaching and quality improvement in the academic geriatric medicine programs. This enhances the marketing for the program.
Partners with Aurora Research Institute when appropriate to support research and clinical trial opportunities.
Serves as the Senior Services representative to administrative and operational leaders across the region. Collaborates with leadership in the resolution of professional conduct concerns of Senior Services clinicians and guides resolution of patient concerns related to their care or clinician interaction.
Works with Advocate Health Governmental Affairs, regulatory agencies, legislators, population health related societies and associations to develop Senior Services standards. Partners with local community and population health leaders to identify community needs and supports solution development.
Licensure, Registration, and/or Certification Required:
- Medicine and Surgery, MD-DO license issued by the state in which the team member practices, and
- Internal Medicine or Family Medicine board certification by AMBS
- Board Certification by ABOM
Education Required:
- Doctorate Degree in Medicine.
Experience Required:
- Typically requires 5 years of of full-time experience in n a family/internal medicine practice setting.
Knowledge, Skills & Abilities Required:
- Thorough knowledge and understanding of current trends, legislation, and regulatory requirements, healthcare delivery models, population health, care management, managed care, ACOs and access to care.
- Demonstrated success in developing positive physician, APC, and leadership relationships and collaborations.
- Skilled in handling crucial conversations.
- Analytical, communication, problem solving and diplomacy skills.
Physical Requirements and Working Conditions:
- Exposed to normal ambulatory and acute care work settings.
- Some travel required.
